(Company Name) urges all prospective employees to provide a complete and detailed occupational and medical history. All information received either prior to employment or anytime during employment will be kept confidential. All employees are required to report any suspected exposures, regardless of the degree to the medical surveillance coordinator. Any unusual physical or psychological conditions should be brought to the physician's attention. (Even the vaguest disturbances or apparent minor complaint such as skin irritation or a headache may be important.)
