Many workplaces contain spaces considered "confined"; some because of their configuration, others may contain high toxic atmospheres, and still others may contain hazards that may asphyxiate or engulf a worker. (Company) has developed this system (see Appendix B) to be used for permitting authorized employees to enter permit spaces and to document the measures taken to protect authorized entrants from permit space hazards. This permit enables the entry supervisor and other personnel involved in the entry operation to keep track of the precautions taken to protect employees. It will also allow authorized entrants to verify that each protective measure has been checked by someone.
EQUIPMENT A complete listing of all safety equipment is in Appendix D of this program. Each employee involved in a permit entry will be trained on the use of all equipment and how to maintain it properly. (Name/Title) will have the overall responsibility of this equipment including proper selection, calibration and maintenance of all testing and monitoring equipment.
